Derek Milligan was a business associate of Gary Windass, who also owed him money.
In August 2019, Gary, feeling guilty over the death of Rana Habeeb in the Underworld roof collapse, aimed to secure a building contract at the factory. Derek, who owed money to Gary, was used as a scapegoat by Gary in exchange for covering his debts.
The following month, Derek took a shine to Gary's ex-girlfriend Izzy Armstrong. Although Gary wasn't happy with this, Derek insisted that he was needed in order for Gary's plans to succeed. After Gary beat up Ryan Connor, Gary told Derek that the same would happen to him if he didn't follow his orders from now on. Derek took the hint, and called things off with Izzy, before fleeing the street.
